Feel free to fill out the intake forms beforehand, located under ‘Forms’ tab. Also bring your driver’s license and insurance card(s).
Do you have same-day appointments?
We are able to accommodate almost all same-day appointments. Please call and ask the staff for availability.
I’d like a 2nd opinion. What should I bring?
Any previous surgeon’s notes, operative reports, or x-rays are helpful for background information.
What are your wait times?
Most patients are seen within twenty minutes of completion of their intake forms Your time is valuable.
What insurance do you accept?
We accepts most private and government insurance including, but not limited to Blue Cross Blue Shield, United, Medcost, Tricare, Cigna, Humana, Aetna, Humana, Medicare and Medicaid. The practice is currently accepting new Medicare patients.
